Brent, Central London, Ealing, Hammersmith and Fulham, Harrow, Hillingdon, Hounslow, and West London Clinical Commissioning Groups (CCGs) have merged as of 1 April 2021 to form North West London CCG. Brent, Central London, Ealing, Hammersmith and Fulham, Harrow, Hillingdon, Hounslow, and West London Clinical Commissioning Groups is transferring to the new CCG – North West London Clinical Commissioning Group on 1 April 2021. The new Clinical Commissioning Group will become the new data controller.

Community health services

Community health services deliver accessible NHS healthcare services to adults and children in convenient places close to their homes and, where necessary, in their homes.

Some of the services on offer include health visiting, physiotherapy, district nursing and school nursing, speech and language therapy, smoking cessation, specialist dentistry, and emergency and out of hours care.

Through assessing, treating, educating and caring for local people, they aim to improve health, enable well-being and enhance the quality of their lives.
