Making your care more convenient, in your own community
18 treatments are now available in a convenient, community setting. If you already receive any of these treatments in a hospital setting, this will save you the trip, because you can now get the care you need in a nearby GP surgery or hub.
This is part of our bold plan to make it easier than ever for you to access a range of treatments and care packages, from blood tests and ECGs to wound care and some mental health services.

How do I access these services in a GP surgery?
If you live in Westminster, Kensington & Chelsea, Hammersmith & Fulham, Hounslow, or Ealing, and currently receive any of these services in a hospital setting, you may be referred to a community service provider by your GP . If you would like to know more or think this scheme may be right for you, please inquire at your registered GP practice.
What does this mean for my care?
We know that patients can be confident in the treatment provided in local surgeries. Most people tell us they trust their GP, and it will be on their GP’s instruction that they will be referred to receive their care either at their own GP surgery, or at another surgery nearby.
The care you receive will be regulated and provided by well-trained experts. You will be referred to the site that specialises in the treatment that you need. This means you will always receive a consistently high quality of care from the most appropriate provider.
With your explicit and informed consent, the practice you are referred to will be able to access your NHS patient record to help them provide you with the best possible care. You will not need to explain to the GP or nurse treating you about any pre-existing conditions, allergies, mental health troubles, prior medication, or any other relevant details, unless you feel a particular need to do so, because they will have this information available.
The data you share with NHS and social care services is used to plan, manage, and deliver your care. It is also used to research new treatments and identify health trends, so we can improve services for everyone. Find out more here about how data sharing works and how you can give or refuse consent.
What does it mean for my local NHS as an organisation?
To help better serve you GPs will be enabled to act as both provider and coordinator of services. In order to do this, GPs across North West London have organised themselves into GP Federations.
